Overview

There are now more patients who have Heart Failure with preserved Ejection Fraction (HFpEF) than those with reduced Ejection Fraction (HFrEF). Making the diagnosis of HFpEF is more challenging than HFrEF as many commonly used screening tools such as physical examination, echocardiography and measurement of natriuretic peptide levels are less sensitive and specific in this population. Most importantly, no therapies have yet been shown to improve major cardiovascular outcome (i.e. reductions in hospitalization rates or lower mortality). Several ongoing studies are evaluating novel treatments for this disorder.

Objectives
At the conclusion of this activity, learners will be able to:

  1. Discuss contemporary approaches to the diagnosis, phenotyping, and management of heart failure with preserved ejection fraction (HFpEF), including the role of imaging, exercise testing, and remote monitoring technologies.
  2. Evaluate emerging and evidence-based therapeutic strategies for HFpEF, including pharmacologic therapies, lifestyle and exercise interventions, obesity management, and precision medicine approaches tailored to individual patient populations.
  3. Identify the impact of associated cardiovascular and systemic comorbidities—including arrhythmias, valve disease, cardio-kidney-metabolic syndrome, and inflammation—on HFpEF progression and patient outcomes.
  4. Review current and evolving research, clinical trials, and multidisciplinary care models that may improve prevention, symptom management, quality of life, and long-term outcomes in patients with HFpEF.
